Plus d&rsquo;un tiers des troubles reconnus et pris en charge sont donc dus \u00e0 cette seule maladie, qui est une cause importante de handicap dans la population. <\/p>\n\n\n\n<p>D&rsquo;un point de vue anatomique, le canal carpien est un \u201ctunnel\u201d ost\u00e9o-fibreux au niveau du poignet dans lequel cheminent des tendons et un nerf : <strong>le nerf m\u00e9dian<\/strong>. Comme l&rsquo;espace \u00e0 l&rsquo;int\u00e9rieur de ce tunnel n&rsquo;est pas extensible \u00e0 volont\u00e9, le nerf peut y \u00eatre atteint ou comprim\u00e9 (m\u00eame si les m\u00e9canismes exacts sont encore en d\u00e9bat), ce qui d\u00e9clenche le syndrome : c\u2019est ce que l\u2019on appelle une <strong>neuropathie<\/strong>. Dans ce cas-l\u00e0, la neuropathie se traduit par des sympt\u00f4mes dans la main, le poignet et \u00e9ventuellement l&rsquo;avant-bras. Notons que deux gros facteurs de risques sont moins connus mais assez bien document\u00e9s : <strong>le tabac et l&rsquo;alcool<\/strong>.<\/p>\n\n\n\n<p><strong>ERRATUM <\/strong>: notons qu&rsquo;en novembre 2022, une m\u00e9ta-analyse contredit le fait qu&rsquo;un lien soit clair entre syndrome du canal carpien et tabagisme : Lampainen K, Hulkkonen S, Ryh\u00e4nen J, Curti S, Shiri R.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9601480\/\" target=\"_blank\" rel=\"noreferrer noopener\"> Is Smoking Associated with Carpal Tunnel Syndrome? A Meta-Analysis<\/a>. Healthcare (Basel). 2022 Oct 11;10(10):1988. doi: 10.3390\/healthcare10101988. PMID: 36292435; PMCID: PMC9601480.<\/p>\n\n\n\n<p>Les auteurs concluent en effet que les r\u00e9sultats des pr\u00e9c\u00e9dentes \u00e9tudes sont dus \u00e0 des facteurs de confusion. En ne retenant que les \u00e9tudes de haute qualit\u00e9, ils ne trouvent donc pas d&rsquo;association. Nous sommes par ailleurs convaincus par la qualit\u00e9 m\u00e9thodologique de cette derni\u00e8re \u00e9tude. <\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Comment le traite-t-on ?<\/h4>\n\n\n\n<p>Actuellement, il est largement r\u00e9pandu que ce syndrome ne se traite que par op\u00e9ration. Si les \u00e9l\u00e9ments dans le canal sont comprim\u00e9s, il \u00ab\u00a0suffit\u00a0\u00bb de le lib\u00e9rer chirurgicalement pour soulager les patients, et c\u2019est vrai. Partiellement. N\u00e9anmoins, parce que l&rsquo;on a une m\u00e9thode efficace pour traiter une affection, nous semblons d\u00e9laisser toutes les autres pistes de soin\u2026 Et pourtant, m\u00eame si ces r\u00e9sultats sont tr\u00e8s peu vulgaris\u00e9s (<a href=\"https:\/\/blog.predictanalyse.com\/index.php\/2021\/11\/29\/les-experts-ont-tort-et-les-articles-sont-faux\/\" target=\"_blank\" rel=\"noreferrer noopener\" title=\"Allons savoir pourquoi\u2026\">Allons savoir pourquoi\u2026<\/a>) , <strong>la recherche semble nous en proposer. <\/strong>Alors si les op\u00e9rations ont des r\u00e9sultats globalement b\u00e9n\u00e9fiques et un haut taux de r\u00e9ussite, quel serait l&rsquo;int\u00e9r\u00eat ?<\/p>\n\n\n\n<p>Premi\u00e8rement, ne banalisons pas une op\u00e9ration qui reste une m\u00e9thode lourde et invasive pour soigner quelqu&rsquo;un. Si nous pouvons nous en passer, tant mieux ! Ensuite, la chirurgie n&rsquo;est qu&rsquo;une ressource de dernier recours, ce qui implique que la maladie peut trainer longtemps avant sa prise en charge. Se reposer sur une alternative en bout de course fait que l&rsquo;on <strong>n\u00e9glige enti\u00e8rement les possibilit\u00e9s pr\u00e9ventives des traitements<\/strong>. Faisons donc le point sur les \u00e9l\u00e9ments scientifiques qui existent et qui peuvent nous apporter d&rsquo;autres options plus conservatrices et moins invasives.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Quels changements proposer ?<\/h4>\n\n\n\n<p>Commen\u00e7ons par pr\u00e9ciser que bon nombre de traitements ont \u00e9t\u00e9 \u00e9tudi\u00e9s, et que l&rsquo;inefficacit\u00e9 de beaucoup d&rsquo;entre eux a \u00e9t\u00e9 d\u00e9montr\u00e9e : citons par exemple l&rsquo;\u00e9lectroth\u00e9rapie, le laser et les ultrasons. Les <strong>attelles <\/strong>de repos semblent avoir un effet, bien que limit\u00e9, plut\u00f4t dans les syndromes peu graves et \u00e0 court terme, ce qui peut s&rsquo;av\u00e9rer une option. M\u00eame si nous ne pouvons pas affirmer que cela \u00e9vitera l&rsquo;op\u00e9ration, c&rsquo;est une possibilit\u00e9 non n\u00e9gligeable en tant que traitement non-invasif (une revue de 2022 r\u00e9sume cela<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9847107\/pdf\/RU-60-157307.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"> ici<\/a>).<\/p>\n\n\n\n<p>Toutefois, certains sujets \u00e0 l&rsquo;\u00e9tude sont plus int\u00e9ressants, comme les th\u00e9rapies manuelles et les exercices <strong>neurodynamiques<\/strong>. Ce type de soin consisterait \u00e0 travailler<strong> sur la mobilit\u00e9 des tissus (du nerf entre autres) dans le but de restaurer la fonction nerveuse<\/strong>. Cela se r\u00e9alise par des mouvements assez simples du membre sup\u00e9rieur qui ne sont ni douloureux ni \u00e9puisants. <\/p>\n\n\n\n<h4 class=\"wp-block-heading\">La piste neurodynamique<\/h4>\n\n\n\n<p>Il est important de pr\u00e9ciser que pour le moment, <a href=\"https:\/\/blog.predictanalyse.com\/index.php\/2022\/01\/31\/la-medecine-5p-le-p-de-preuves\/\">les r\u00e9sultats de recherche<\/a> sont encore trop disparates et contradictoires pour d\u00e9terminer les protocoles \u00e0 suivre et les tailles d&rsquo;effet de ces approches. Mais si la trop faible quantit\u00e9 d&rsquo;\u00e9tudes ne nous permet pas encore de trancher le d\u00e9bat, la balance penche clairement en faveur de l&rsquo;introduction de ces proc\u00e9dures non invasives, qui ne pr\u00e9sentent pas de risque et qui peuvent \u00eatre effectu\u00e9es \u00e0 n&rsquo;importe quel stade de l&rsquo;\u00e9volution de la maladie. En effet, des r\u00e9sultats int\u00e9ressants sont observ\u00e9s pour <strong>diminuer les sympt\u00f4mes neuropathiques, pour r\u00e9duire les douleurs, pour retrouver des fonctionnalit\u00e9s et de la force dans la main<\/strong>. Si ces m\u00e9thodes ne peuvent pas \u00eatre appliqu\u00e9es \u00e0 tous les cas, car tous les syndromes du canal carpien ne se valent pas quant \u00e0 leur \u00e9tiologie, leur traitement et leur pronostic, ils sont quand m\u00eame une possibilit\u00e9 pour beaucoup, principalement dans les cas d\u00e9butants.<\/p>\n\n\n\n<p>Avec un court apprentissage, les patients pourraient m\u00eame am\u00e9liorer leur pronostic par des exercices simples et facilement r\u00e9alisables sans l&rsquo;aide d&rsquo;un professionnel de sant\u00e9. Si on consid\u00e8re les <strong>130000 personnes qui se font op\u00e9rer chaque ann\u00e9e en France<\/strong>, cela aurait un impact soci\u00e9tal non-n\u00e9gligeable, m\u00eame si l&rsquo;efficacit\u00e9 n&rsquo;\u00e9tait \u00ab\u00a0que\u00a0\u00bb de 50%. Bien s\u00fbr, ce chiffre est choisi totalement arbitrairement mais les possibilit\u00e9s qu&rsquo;ouvre ce type de traitement m\u00e9rite que l&rsquo;on s&rsquo;y attarde. Par ailleurs, nous n&rsquo;avons malheureusement pas de donn\u00e9es sur l&rsquo;efficience de ces mesures \u00e0 titre pr\u00e9ventif &#8211; qui pourraient \u00eatre propos\u00e9es pour des profils \u00e0 risque &#8211; puisqu&rsquo;aucune \u00e9tude n&rsquo;a jamais \u00e9t\u00e9 r\u00e9alis\u00e9e en ce sens&#8230;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Conclusion<\/h4>\n\n\n\n<p>Si la litt\u00e9rature nous informe que de nouveaux traitements vont sans doute prendre de plus en plus de place dans les protocoles de suivi des patients, nous ne sommes pas encore en mesure de correctement d\u00e9terminer \u00e0 quel point cela peut \u00eatre <strong>b\u00e9n\u00e9fique<\/strong>, et c&rsquo;est dommage ! Il est temps de vulgariser le fait que la prise en charge du syndrome du canal carpien n&rsquo;est pas r\u00e9serv\u00e9e aux chirurgiens.
